*New Proof-of-Age Requirement:
1. Copy of State-issued Birth Certificate;
2. Copy of passport;
3. Copy of Driver's License;
4. Letter from child's school stating legal name and date of birth;
5. Alternatives need discussion. (Copies shredded after.)
